Lay summary
This project focuses on improving general practice services to meet the needs of individuals with complex health needs and diabetes and living in low decile communities. The award enables the applicant to receive mentoring and undertake a placement at a very low-cost access general practice who provide services to an ethnically diverse population with high rates of diabetes and complex needs. Extending previous work, this placement will continue development of a practice-research partnership aimed at understanding the needs of this practice and designing and undertaking research to address real-world health delivery issues. Routinely collected data from the practice will be used develop a detailed summary of the practice population and comprehensive case studies of care provision will be used to document and understand the current work of the entire practice team, relevant resource implications and identify new innovations for service delivery in this context.
