Lay summary
The current priorities within the New Zealand healthcare system include access, timeliness of service, an emphasis on quality, the need for flexibility and a focus on non-communicable diseases. With experience working in inter-disciplinary teams across secondary and primary care sectors, many hauora haumi/allied health professionals are uniquely experienced in supporting patient transitions between care settings, a critical strength that can support greater coordination and continuity in patient journeys. There is an opportunity to design health services that make better use of hauora haumi to achieve health objectives for efficiency, quality, equity and sustainability in health and disability services. The research team is highly experienced in using collaborative design approaches between researchers, health professionals and the community, which are critical to creating sustainable care solutions. This research will co-design research informed and focused allied health interprofessional clinical innovations to support sustainable service delivery to priority groups across Aotearoa.
