Lay summary
The placement research activity will focus on developing and evaluating a model of diabetes eye care services from a primary care perspective. This work will be co-designed with key stakeholders and informed by real-world insights from within the primary care sector. In collaboration with the team at the National Hauora Coalition (NHC), the placement host organisation, I will facilitate a series of design workshops that provide safe, inclusive spaces for stakeholders to share their experiences, needs, and aspirations. These sessions will guide the creation of new service pathways that are culturally responsive and equity-focused. Following the co-design phase, we will pilot the model of care and conduct a comprehensive evaluation. This process will not only inform service improvements but also lay the groundwork for ongoing collaboration with NHC to extend integrated care approaches to other sensory impairment conditions associated with diabetes.
