Stroke is the one of the top causes of death and disability worldwide. Patients with stroke and transient ischaemic attack (TIA) are at increased risk of recurrent (more disabling) stroke. The rate of recurrent strokes has not declined significantly in NZ over the last 3 decades. Currently there is no individually tailored stroke-specific tool for clinicians to assess their patients' risk and provide recommendations at discharge and follow-up. Similarly, stroke and TIA patients have no way to monitor and self-manage their post-discharge lifestyle and medications, tailored for their own needs. This study aims to test two novel digital tools to address these urgent needs for secondary stroke prevention: PreventS-MD app for stroke physicians combined with the Stroke Riskometer app for stroke patients. We hypothesise that the combined use of these two digital tools will result in significant improvements in management of risk factors, particularly blood pressure, and reduce recurrent stroke.
