Lay summary
This grant will look at the importance in supporting primary care to deliver new medicines for type 2 diabetes like Empagliflozin, in a way that connects with males aged under 40, in particular Māori and whānau together. This cohort has the potential to benefit most from early intervention. This activation activity will give a real-time focus on understanding current practice for supporting diabetes medications concordance. The goal of the activation activity is to address ways we can deliver the best possible services for people in our communities who are currently not being supported well enough. It is intended to collect the voices of both patients and those involved in their care to provide a basis for co-designing interventions in a meaningful way. By reducing the risk that comes with diabetes, we can reduce inequity, particularly within the younger age group.
