This research will evaluate the effectiveness of a range of Better, Sooner, More Convenient (BSMC) health initiatives in improving the integration of health care services and improving patient experience of health care in MidCentral DHB. The overall aim is to provide rigorous evaluation research examining the effectiveness of key health care integration initiatives, the barriers and facilitators to their implementation, and their impact on patient experience of health care. We will focus particularly on evaluating (1) chronic condition management, (2) comprehensive health assessment and care-plan development, and (3)a Shared Care Electronic Health Record. We will also develop an evaluation framework and tool-kit of instruments for use in future BSMC evaluation research.
