National Application Form
The National Application Form for Ethical Approval of a Research Project (NAF-2009-v1) is to be used for all applications considered by the Health and Disability Ethics Committees. This includes:
Part 1 – Basic Information
Part 2 – Ethical Principles
Part 3 – General
Part 4 – Declarations
Part 5 – Use of Human tissue
Part 6 – Genetic Section
Part 7 – When a participant is unable to make an informed choice
Part 8 - Xenotransplantation Clinical Trial Section
Parts 5, 6, 7 and 8 only need to be completed if relevant.
Guidelines
The Guidelines for Completion of the National Application Form for Ethical Approval of a Research Project (NAFG-2009-v1) are to be read before completing the National Application Form to ensure that the questions are answered appropriately.
